Obesity

579
The Body Mass Index (BMI) is a numerical value derived from a person's weight and height, used to categorize individuals into weight ranges. It is calculated using the formula: weight in kilograms divided by height in meters squared. Obesity is a health condition characterized by excessive accumulation of adipose tissue that poses health risks, often diagnosed with a BMI ≥ 30. This excess fat storage occurs when surplus dietary calories are converted into triglycerides and stored in...

Community Based Intervention

87
Community-based interventions in mental health represent a paradigm shift from institution-centered care to treatments embedded within the fabric of local communities. By prioritizing inclusion and leveraging existing societal structures, this approach fosters a supportive environment conducive to addressing mental health challenges while promoting individual dignity and agency.

Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy

389
Various diagnostic tests are employed in the diagnostic process for Inflammatory Bowel Disease (IBD), particularly to differentiate between Crohn's disease and ulcerative colitis.
Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...

Binge Eating Disorders

154
Binge eating disorder is a significant mental health condition characterized by recurrent episodes of excessive food consumption within a short period, accompanied by a perceived loss of control over eating behavior. Unlike occasional overeating, binge eating disorder is marked by distressing emotions such as guilt, shame, and anxiety following binge episodes. The disorder affects individuals across different ages and backgrounds, with profound implications for physical and psychological...

Acute Coronary Syndrome IV: Interprofessional Care

22
IntroductionThe management of Acute Coronary Syndrome (ACS) aims to minimize myocardial damage, preserve myocardial function, and prevent complications.Initial ManagementInpatient management involves continuous cardiac monitoring, preferably in an ICU, focusing on blood pressure, serum sodium, potassium, and creatinine levels, and urine output. Ongoing pharmacologic management is crucial for stabilizing the patient.Supplemental Oxygen: Administer supplemental oxygen if oxygen saturation is...

Summary

Multidisciplinary pediatric obesity clinics effectively reduce BMI Z-score during treatment and maintain these gains long-term. Early referral age is key for sustained pediatric obesity management.

Area of Science:

  • Pediatric Endocrinology
  • Public Health
  • Clinical Nutrition

Background:

  • Pediatric obesity management in outpatient settings presents challenges due to time demands.
  • Multidisciplinary clinic programs show initial success in reducing BMI, but long-term sustainability is less understood.

Purpose of the Study:

  • To evaluate the sustained post-treatment outcomes for children attending a multidisciplinary pediatric obesity clinic (MPOC).

Main Methods:

  • Retrospective chart review of 472 MPOC patients (2008-2016) with at least two visits.
  • Primary outcome: Change in BMI Z-score (BMIZ) at 1 and 2 years post-intervention.
  • Multivariate regression identified predictors of BMIZ change.

Main Results:

  • Mean BMIZ significantly decreased during intervention and was maintained at 1- and 2-year follow-ups.
  • Younger children (ages 3-5) showed further BMIZ reduction at 1 year.
  • Earlier referral age was the sole significant predictor of BMIZ change.

Conclusions:

  • Outpatient multidisciplinary intervention effectively reduces or stabilizes pediatric BMIZ long-term.
  • Early referral enhances sustained treatment outcomes for pediatric obesity.
  • While primary prevention is ideal, multidisciplinary clinics offer effective sustained treatment options.
