Granular cell tumors (GCTs) are rare spinal canal tumors. Complete surgical resection is the preferred treatment for these benign nerve sheath tumors to prevent recurrence.

Background:
- Granular cell tumors (GCTs) are uncommon peripheral nerve sheath tumors originating from Schwann cells.
- While GCTs can occur anywhere in the body, their presence within the spinal canal is exceptionally rare.
Observation:
- A 49-year-old male experienced a burning sensation in his left knee.
- Lumbar spine MRI revealed an intradural extramedullary tumor at the L3-L4 level.
- The patient underwent complete surgical resection of the tumor.
Findings:
- Histopathological examination confirmed the lesion as a benign Granular Cell Tumor (GCT).
- Post-surgery, the patient's L3 root-related burning sensation significantly improved.
Implications:
- Spinal GCTs, though rare, can present as intradural extramedullary masses.
- Complete surgical resection is the recommended treatment to minimize the risk of recurrence.
- Incomplete resection may necessitate further treatment, such as radiation therapy.
