Local Anesthetics: Clinical Application as Epidural Anesthesia

482
Epidural anesthetics are administered in the fat-filled epidural space, the outermost part of the spinal canal. This technique is commonly employed for pain management and anesthesia during lower abdomen and pelvis surgeries or labor and delivery.
Since epidural anesthetics can be infused through an epidural catheter, all types of drugs, including short-acting ones, can be administered. Chloroprocaine and lidocaine are examples of short and long-duration anesthetics, respectively. Bupivacaine...

Area of Science:

  • Neurology
  • Neurosurgery
  • Trauma Surgery

Background:

  • Post-traumatic spinal epidural hematoma (PSEH) is a rare clinical presentation following trauma.
  • It involves a collection of blood within the epidural space of the spinal canal.

Observation:

  • A case of a 55-year-old patient who sustained a cervical epidural hematoma (CEH) from C3 to C5 after a traffic accident is presented.
  • The patient presented one hour post-trauma with symptoms requiring immediate intervention.

Findings:

  • Computed Tomography (CT) scan revealed a compressive epidural hematoma extending from C3 to C5.
  • Surgical evacuation of the hematoma was successfully performed via a posterior approach.

Implications:

  • Cervical epidural hematoma is a medical emergency requiring rapid diagnosis and management.
  • Effective treatment necessitates multidisciplinary collaboration among Intensive Care Unit (ICU) specialists, neurosurgeons, neuroradiologists, and physiotherapists.
  • Optimal patient outcomes depend on timely intervention and comprehensive follow-up care.
