A new web-based advance care planning (ACP) program effectively increased patient engagement and readiness for discussing future treatment preferences. This evidence-based tool was found to be usable and satisfactory, supporting initial steps in advance care planning.
Area of Science:
- Digital Health
- Patient Engagement
- Health Informatics
Background:
- Web-based advance care planning (ACP) programs can aid patients in considering and communicating future care preferences.
- However, accessible and evidence-based ACP programs are limited.
Purpose of the Study:
- To develop and evaluate an evidence-based, interactive web-based ACP program.
- The program aims to guide users through thinking, discussing, and recording treatment and care preferences.
Main Methods:
- Developed and pilot-tested the "Explore your preferences for treatment and care" program.
- Evaluated ACP engagement, usability, and satisfaction in 147 individuals with chronic diseases before and 2 months after program completion.
Main Results:
- ACP engagement scores increased significantly (2.8 to 3.0), as did contemplation (2.6 to 2.8) and readiness (2.2 to 2.5).
- The program demonstrated good usability (mean 70/100), attractiveness (3.8/5), and comprehensibility (4.2/5).
- No significant changes were observed in knowledge or self-efficacy regarding ACP.
Conclusions:
- An evidence-based, interactive web-based ACP program was successfully developed and evaluated.
- The program supports initial steps in advance care planning and preference reflection, proving usable and satisfactory.
- Healthcare professionals can utilize this tool to initiate ACP discussions and enhance patient awareness.
