Summary

Fungal keratitis, a common cause of visual impairment in India, often requires penetrating keratoplasty when medical treatment fails. Early surgical intervention is key to saving the eye and restoring vision.

Background:

  • Infective keratitis is a leading cause of visual impairment, particularly in rural India.
  • Fungal keratitis presents a poor prognosis due to delayed diagnosis and limited effective treatments.

Observation:

  • A 56-year-old female presented with worsening symptoms of a fungal corneal ulcer despite two months of antifungal therapy.
  • Examination revealed a descemetocele with impending corneal perforation.

Findings:

  • Therapeutic penetrating keratoplasty was recommended for the fungal corneal ulcer.
  • Penetrating keratoplasty effectively replaces diseased tissue, reduces infection, and restores ocular integrity.

Implications:

  • Penetrating keratoplasty is crucial for refractory keratitis, impending or actual perforation.
