Summary

This case report details a rare instance of monkeypox virus causing epithelial keratitis. PCR confirmed monkeypox DNA in the cornea, highlighting a new ocular manifestation of the virus.

Background:

  • Monkeypox virus (MPXV) primarily causes mucocutaneous lesions.
  • Ocular manifestations of MPXV are uncommon, with previous reports focusing on blepharitis and conjunctivitis.

Observation:

  • A 45-year-old male developed epithelial keratitis with a corneal ulcer 20 days after MPXV diagnosis.
  • Polymerase chain reaction (PCR) analysis of corneal epithelium confirmed the presence of human monkeypox virus DNA.

Findings:

  • The patient received ganciclovir gel, povidone iodine, moxifloxacin eye drops, and oral tecovirimat.
  • A therapeutic contact lens was used, and the corneal defect healed within 20 days, leaving minimal subepithelial haze and good visual acuity (0.8).

Implications:

  • This case confirms MPXV can cause epithelial keratitis, an uncommon but significant ocular complication.
  • Early diagnosis and appropriate antiviral treatment are crucial for managing MPXV-related ocular disease.
  • Further research is needed to understand the pathogenesis and long-term visual outcomes of MPXV keratitis.
