Moderate-to-severe malnutrition significantly increases the risk of left ventricular (LV) remodeling in heart failure patients. This study highlights the importance of nutritional status in predicting adverse cardiac events and LV remodeling.

- LV remodeling was defined as a ≥10% decrease in LV ejection fraction post-discharge. Associations were analyzed using logistic regression.

- 9.9% of patients (712) experienced LV remodeling.
- The moderate-severe malnutrition group had a significantly higher incidence of LV remodeling (12.9% vs. 9.5%, p=0.002).
- Moderate-severe malnutrition was independently associated with a 1.69-fold increased risk of LV remodeling (OR: 1.69, CI: 1.32-2.16).

- Moderate-to-severe malnutrition is a significant risk factor for LV remodeling in heart failure patients.
- Approximately 12% of patients with moderate-severe malnutrition developed LV remodeling.
- Further prospective studies are warranted to explore nutrition-focused interventions for patients with LV remodeling.
