Summary

Myocardial norepinephrine depletion strongly correlates with left ventricular dysfunction in idiopathic dilated cardiomyopathy. This relationship is independent of muscle fiber hypertrophy, interstitial volume, and sympathetic tone.

Background:

  • Idiopathic dilated cardiomyopathy (IDC) is characterized by left ventricular dysfunction.
  • Catecholamines play a role in cardiac function and dysfunction.
  • Morphological changes in the myocardium are associated with IDC.

Purpose of the Study:

  • To investigate the relationship between left ventricular function, myocardial and plasma catecholamine levels, and cardiac morphology.
  • To determine if myocardial catecholamine depletion is linked to left ventricular dysfunction in IDC.

Main Methods:

  • Studied 20 patients (15 with IDC, 5 healthy controls).
  • Performed right ventricular endomyocardial biopsy to assess myocardial catecholamine content, muscle fiber thickness, and interstitial volume.
  • Measured plasma catecholamine concentrations and left ventricular ejection fraction via cineangiography.

Main Results:

  • Left ventricular ejection fraction correlated positively with myocardial norepinephrine content (r=0.80) and negatively with plasma epinephrine (r=-0.53) and muscle fiber thickness (r=-0.50).
  • Myocardial norepinephrine concentration was negatively correlated with plasma epinephrine (r=-0.62).
  • Multiple regression analysis showed myocardial norepinephrine depletion strongly correlates with left ventricular dysfunction, independent of other variables.

Conclusions:

  • Myocardial norepinephrine depletion is a significant correlate of left ventricular dysfunction in idiopathic dilated cardiomyopathy.
  • This depletion is independent of muscle fiber hypertrophy, interstitial volume, and elevated sympathetic tone.
