Background:

  • Diffuse midline gliomas (DMGs) are rare primary brain tumors.
  • DMGs are typically diagnosed in children and adolescents.
  • The H3F3A K27M mutation is a common genetic marker in DMGs, regardless of patient age.

Observation:

  • A 42-year-old male patient presented with a brainstem glioma.
  • Routine genetic analysis for H3F3A K27M mutation was performed.
  • The tumor was found to harbor an IDH mutation instead of the typical H3F3A K27M mutation.

Findings:

  • Identification of an isocitrate dehydrogenase (IDH)-mutant brainstem glioma in an adult.
  • This represents the first documented case of an IDH-mutant brainstem glioma in an adult patient.
  • The genetic profile differed from the commonly screened H3F3A K27M mutation.

Implications:

  • This finding expands the known spectrum of adult brainstem gliomas.
  • It highlights the importance of comprehensive genetic profiling beyond common mutations.
  • Further research is needed to understand the clinical behavior and treatment of IDH-mutant brainstem gliomas in adults.
