Endoscopic Studies I: Bronchoscopy and Thoracoscopy

249
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due...

Endoscopic Procedures V: ERCP

703
Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

148
Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Summary

Laparoscopic common bile duct exploration (LCBDE) is underused due to training and instrument barriers. A disposable bronchoscope enabled successful LCBDE in a patient with Roux-en-Y gastric bypass, offering a novel visualization alternative.

Area of Science:

  • Minimally Invasive Surgery
  • Gastroenterology
  • Surgical Innovation

Background:

  • Laparoscopic common bile duct exploration (LCBDE) is underutilized for common bile duct (CBD) stones.
  • Barriers include lack of training and specialized instruments.
  • Anatomical challenges in Roux-en-Y gastric bypass patients make ERCP difficult.

Purpose of the Study:

  • To present a case of successful LCBDE using a disposable bronchoscope.
  • To highlight a potential solution for instrument unavailability in LCBDE.
  • To demonstrate an alternative visualization method for CBD stone management.

Main Methods:

  • Case report of a 61-year-old male with Roux-en-Y gastric bypass and a CBD stone.
  • Utilized a disposable bronchoscope for direct visualization during LCBDE.
  • Performed laparoscopic common bile duct exploration for stone retrieval.

Main Results:

  • Successful retrieval of the CBD stone was achieved.
  • The disposable bronchoscope provided adequate visualization of the biliary tract.
  • LCBDE was effectively performed despite anatomical challenges.

Conclusions:

  • Disposable bronchoscopes can serve as a viable alternative for choledochoscopy in LCBDE.
  • This approach may overcome instrument availability barriers.
  • Offers a potential solution for managing CBD stones in complex surgical anatomy.
