Summary

Severe acute streptococcal glomerulonephritis in children can cause serious symptoms like kidney failure and hypertension. Prompt management led to full recovery in all four reported cases.

Background:

  • Acute streptococcal glomerulonephritis (ASGN) is a post-infectious kidney disease.
  • Severe presentations in children require careful monitoring and management.

Observation:

  • Four children presented with severe ASGN.
  • Prominent symptoms included oliguria, renal failure, hypertension, nephrotic syndrome, anemia, and gastrointestinal bleeding.

Findings:

  • Renal biopsy showed diffuse endocapillary glomerulonephritis with crescents and vasculitis.
  • One patient exhibited multiorgan vasculitis.

Implications:

  • Conventional management, including anticoagulants and immunosuppressants, resulted in complete recovery for all patients.
  • Highlights the potential severity and successful treatment of pediatric ASGN.
