Summary

Primary hepatic paragangliomas are rare tumors. This case highlights the importance of screening for these tumors in pregnant patients with refractory hypertension, with laparoscopic surgery being a safe treatment option.

Background:

  • Paragangliomas are rare, highly vascular tumors originating from paraganglia.
  • While commonly found in the retroperitoneum and head/neck, hepatic paragangliomas are exceptionally uncommon.
  • These tumors can be functional, secreting catecholamines and causing significant clinical manifestations.

Observation:

  • A 34-year-old female presented with refractory hypertension during pregnancy, with no prior medical history.
  • Imaging revealed a large solitary lesion in the left lobe of the liver.
  • Elevated plasma-free catecholamines confirmed a functioning tumor.

Findings:

  • The patient underwent successful laparoscopic left hemihepatectomy for the hepatic paraganglioma.
  • Histopathological analysis confirmed the diagnosis with characteristic immunohistochemical staining.
  • The patient experienced an uneventful recovery and remained recurrence-free at 1-year follow-up.

Implications:

  • Refractory hypertension in pregnancy warrants screening for hepatic paraganglioma via ultrasound and catecholamine testing.
  • Laparoscopic surgery is a safe and feasible approach for treating select hepatic paragangliomas.
  • Early diagnosis and surgical intervention are crucial for managing this rare endocrine tumor.
