Observation:

  • An 11-year-old girl presented with a thigh mass initially diagnosed as a localized AVM via angiography.
  • The mass showed increased tenderness and size, prompting surgical excision.

Findings:

  • Histologic examination revealed the thigh mass was an alveolar soft-part sarcoma, not an AVM.
  • This rare sarcoma can mimic vascular lesions on initial imaging.

Implications:

  • Alveolar soft-part sarcoma can be misdiagnosed as AVM, potentially leading to conservative management instead of necessary oncologic treatment.
  • Awareness of this diagnostic pitfall is critical for clinicians managing pediatric vascular lesions.
