Abstract

Insights

Early seizures after revascularization in moyamoya disease (MMD) are linked to prior and new brain infarctions. These findings highlight key risk factors for postoperative seizure development in MMD patients.

Area of Science:

  • Neurology
  • Neurosurgery
  • Vascular Neurology

Background:

  • Moyamoya disease (MMD) is a rare cerebrovascular disorder.
  • Revascularization surgery is a common treatment for MMD.
  • Early seizures are a potential complication following revascularization in MMD patients.

Purpose of the Study:

  • To identify risk factors for early seizure development after revascularization in moyamoya disease patients.
  • To analyze predictors of early postoperative seizures in this patient cohort.

Main Methods:

  • Retrospective analysis of 298 MMD patients diagnosed between 2015 and 2018.
  • Identification of patients who experienced seizures within one week post-revascularization.
  • Logistic regression analysis to determine significant predictors of early seizure.

Main Results:

  • Fifteen patients (5%) developed seizures within one week of revascularization.
  • Preoperative cerebral infarction (OR: 4.08) and postoperative infarction (OR: 12.89) were significant risk factors for early seizure.
  • Age and infarct side were not significantly associated with early seizure development.

Conclusions:

  • A history of preoperative infarction is an independent risk factor for early seizure post-revascularization in MMD.
  • New postoperative infarction is also an independent risk factor for early seizure in MMD patients.
  • These findings emphasize the importance of considering infarction history in managing MMD patients undergoing revascularization.

Seizures: Classification

Epilepsy is primarily characterized by unpredictable seizures, either provoked by an identifiable factor, such as injury or illness, or unprovoked, occurring spontaneously without apparent cause.
Seizures are typically classified into two main categories: focal and generalized seizures.
Focal Seizures
Focal seizures originate from specific regions of the brain. These seizures are further sub-classified into two types:

Aneurysm II: Clinical Manifestations and Diagnostic Studies

Thoracic, aortic arch and abdominal aneurysms are significant vascular conditions that can present with various clinical manifestations and lead to serious complications. Understanding these manifestations and the appropriate diagnostic studies is essential for effective management and treatment.Thoracic Aortic AneurysmsThoracic aortic aneurysms often remain asymptomatic until they reach a size that impinges on adjacent structures. They typically cause deep, diffuse chest pain that radiates to...

Ischemic Heart Disease: Overview01:17

Ischemic Heart Disease: Overview

Ischemic heart disease occurs when the heart's blood supply dwindles, causing an ominous lack of oxygen and nutrients. This deficiency, stemming from reduced or obstructed blood flow, spells danger, leading to heart muscle damage and dysfunction.
Atherosclerosis, the primary malefactor, orchestrates this dangerous condition. It manifests as the accumulation of fatty deposits, akin to insidious plaques, within arterial walls. As time elapses, these plaques metamorphose, hardening and...
