A rare tongue abscess in a 50-year-old male was successfully treated with incision and drainage. Prompt diagnosis and intervention are crucial for managing this condition and preventing airway compromise.

- A 50-year-old male presented with a three-day history of severe tongue pain, swelling, odynophagia, trismus, dyspnea, and fever.
- Physical examination revealed a fluctuant, erythematous swelling on the left anterolateral tongue.

- Diagnosis of tongue abscess confirmed.
- Incision and drainage under general anesthesia yielded 30mL of pus.
- Post-operative antibiotics were administered, and the patient was discharged after two days.

- Empirical antibiotic coverage should include gram-positive, gram-negative, and anaerobic organisms.
