  • 68Ga-DOTA-FAPI-04 is a novel PET/CT tracer targeting FAP, primarily investigated for cancer imaging.
  • Distinguishing benign conditions from malignancies can be challenging with novel imaging agents.

Observation:

  • A 50-year-old male presented with symptoms suggestive of lung cancer.
  • Initial CT imaging revealed a lung mass with enlarged lymph nodes, consistent with malignancy.
  • 68Ga-DOTA-FAPI-04 PET/CT demonstrated intense tracer uptake in the lung mass and mild uptake in lymph nodes.

Findings:

  • Pathological examination definitively diagnosed the lung mass as tuberculous granuloma.
  • The intense FAPI tracer uptake in the granuloma mimicked findings typically associated with lung cancer.
  • This case highlights a potential pitfall of 68Ga-DOTA-FAPI-04 PET/CT in endemic tuberculosis regions.

Implications:

  • Novel PET/CT tracers targeting FAP may exhibit uptake in non-malignant conditions like tuberculosis.
  • Careful clinicopathological correlation is crucial when interpreting 68Ga-DOTA-FAPI-04 PET/CT findings.
  • Further research is needed to understand FAP expression in inflammatory and infectious lung diseases.
