  • A patient with a Mainz II ileal pouch-anal anastomosis for muscle invasive bladder cancer developed left-sided hydronephrosis.

  • Diagnostic workup revealed that the left ureteral neo-ostium had been inadvertently removed and clipped during colonoscopy, mistaking it for a polyp.
  • This led to complete obstruction of urine outflow from the left kidney.

  • Urine outflow was successfully restored using a rendezvous technique, combining antegrade ureteroscopy and colonoscopy.
  • A biliary stent was placed into the left neo-ostium to facilitate urine drainage after initial nephrostomy placement.

Implications:

  • This case highlights a rare but serious complication of colonoscopy in patients with urinary diversions.
  • The successful use of the rendezvous technique and biliary stent demonstrates an effective management strategy for iatrogenic ureteral obstruction.
