Summary

A short duration of maxillomandibular fixation (MMF) may reduce inflammatory complications after mandibular fracture surgery. This study found a lower complication rate in patients with MMF for less than two weeks.

Background:

  • Open reduction and internal fixation (ORIF) is a common treatment for mandibular angle fractures.
  • Postoperative inflammatory complications (POICs) can occur after ORIF.
  • The duration of maxillomandibular fixation (MMF) may influence the rate of POICs.

Purpose of the Study:

  • To determine if a short duration of postoperative MMF (less than 2 weeks) is associated with a reduced rate of POICs in patients undergoing ORIF for mandibular angle fractures.

Main Methods:

  • Retrospective cohort study of 307 patients treated with ORIF for mandibular angle fractures.
  • Primary predictor: short duration of MMF (< 2 weeks).
  • Outcome: presence of POICs. Statistical analysis included Fisher's exact tests, Wilcox rank-sum tests, and multivariable logistic regression.

Main Results:

  • Patients with short-duration MMF had an 8.3% POIC rate versus 18.2% for no MMF (P=0.08).
  • Adjusted analysis showed a significant decrease in POIC risk with short-duration MMF (aOR=0.32, 95% CI=0.11-0.97).
  • Inferior border rigid fixation was associated with decreased POIC risk compared to other plating types.

Conclusions:

  • A short duration of postoperative MMF may reduce POICs following ORIF of mandibular angle fractures.
  • This finding supports optimizing MMF duration to minimize complications.
