Summary

Left ventricular (LV) segmental behavior varies significantly during the cardiac cycle. Apical segments show greater shortening than basal segments during ejection, regardless of loading or inotropic conditions.

Area of Science:

  • Cardiovascular Physiology
  • Cardiac Mechanics
  • Biomedical Engineering

Background:

  • Understanding regional differences in left ventricular (LV) function is crucial for diagnosing and managing cardiac diseases.
  • Previous studies have explored LV mechanics, but detailed analysis of subendocardial segmental behavior across the cardiac cycle under varying conditions is limited.

Purpose of the Study:

  • To analyze left ventricular (LV) subendocardial segmental behavior throughout the cardiac cycle.
  • To investigate the impact of different loading and inotropic conditions on regional LV function.
  • To determine the relationship between apical and basal segmental shortening during various hemodynamic states.

Main Methods:

  • Utilized ultrasonic crystals implanted in basal (B) and apical (A) LV regions of six conscious dogs.

Related Experiment Videos

  • Monitored LV pressure with a micromanometer and aortic pressure with a cuff occluder.
  • Analyzed segmental behavior during isovolumic contraction, ejection, and isovolumic relaxation under controlled conditions.
  • Main Results:

    • Significant variations in apical (A) and basal (B) segmental behavior were observed during isovolumic contraction and relaxation.
    • A consistent linear relationship between A and B segmental shortening was found during ejection (slope=1.49).
    • Apical segments exhibited greater shortening than basal segments (34.6% vs. 25.0%), irrespective of normalization or altered loading/inotropic states.

    Conclusions:

    • Regional stress distribution in the LV accounts for greater apical than basal shortening during ejection.
    • The observed linear relationship during ejection is robust across different loading and inotropic conditions.
    • Inconsistent regional behavior, especially during isovolumic phases, precludes inferring ventricular volume from limited dimensional measurements.
