Purpose of the Study:

  • To investigate factors associated with the presence and severity of bronchiectasis in patients with severe COPD.

Main Methods:

  • An observational, cross-sectional study comparing severe COPD patients with and without bronchiectasis.
  • Evaluations included spirometry, 6-minute walk test (6MWT), volumetric capnography (VCap), and high-resolution computed tomography (CT) with scoring.
  • Multiple linear and logistic regression analyses were used to identify associated factors.

Main Results:

  • No significant clinical differences were found between groups, except lower smoking load in the bronchiectasis group.
  • Patients with bronchiectasis showed higher ΔSpO2 during 6MWT and lower FVC.
  • Positive sputum isolates (OR 4.78) and higher 6MWT distance/Slp3 (VCap) were associated with increased likelihood of bronchiectasis.

Conclusions:

  • Higher CT-based bronchiectasis scores in COPD patients are linked to worse lung function.
  • Increased oxygen desaturation during exercise and positive sputum cultures are associated with bronchiectasis in severe COPD.
