Summary

Tracheostomy granulation tissue is characterized by a dominant M2 macrophage phenotype. Further research is needed to understand their role in inflammation and develop targeted treatments for this common complication.

Background:

  • Tracheostomy-associated granulation tissue is a common complication resulting from chronic mucosal irritation.
  • The specific immune cell populations, particularly monocytes and macrophages, within this tissue are not well-defined.

Purpose of the Study:

  • To characterize the myeloid cell population, focusing on monocytes and macrophages, within tracheostomy-associated granulation tissue.

Main Methods:

  • Biopsies from 8 patients with tracheostomy-induced granulation tissue were analyzed.
  • Flow cytometry, quantitative real-time polymerase chain reaction (qRT-PCR), and immunohistochemistry were employed.
  • Cell populations were compared to tracheal autopsy control specimens.

Main Results:

  • Macrophages and monocytes constituted 23.2% of the granulation tissue cell population.
  • The M2 macrophage phenotype (CD206+) was dominant (77%), significantly increased compared to the M1 phenotype.
  • Classical monocytes were elevated in granulation tissue compared to controls, with M2 gene expression (Arg1/CD206) also increased.

Conclusions:

  • M2 macrophages are the predominant macrophage phenotype in tracheostomy-associated granulation tissue.
  • The pro-inflammatory role of M2 macrophages in this context requires further investigation.
  • Understanding these cellular dynamics may lead to novel therapeutic strategies for granulation tissue.
