Pembrolizumab, a PD-1/PD-L1 inhibitor, can cause rare immune-related adverse effects. A unique syndrome of hypercalcemia, acute kidney injury, and respiratory failure, responsive to steroids, suggests pembrolizumab immunotoxicity.
