Summary

Headaches are uncommon in cervical spondylosis and share features with tension headaches. This suggests secondary muscle contraction may cause headaches in cervical spondylosis patients.

Area of Science:

  • Neurology
  • Pain Management

Background:

  • Cervical spondylosis can cause neurological symptoms, but headache incidence and characteristics are not well-defined.
  • Tension-type headaches are common, and their features are well-established.

Purpose of the Study:

  • To compare the incidence and qualitative features of headaches in patients with cervical spondylosis and neurological disability against those with tension headaches.
  • To identify any distinguishing characteristics between headaches associated with cervical spondylosis and tension headaches.

Main Methods:

  • A retrospective review of 59 documented cases of cervical spondylosis with neurological disability.
  • Comparison of headache characteristics (location, qualitative features) in 5 patients with cervical spondylosis headaches and 50 patients with tension headaches.

Main Results:

  • Headaches were present in only 5 out of 59 reviewed cases of cervical spondylosis.
  • No significant differences were observed in the location or qualitative features of headaches between the cervical spondylosis group and the tension headache group.

Conclusions:

  • The incidence of headache in cervical spondylosis appears low.
  • Headaches in cervical spondylosis lack distinguishing features compared to tension headaches, suggesting similar pathogenesis.
  • Secondary muscle contraction is proposed as the underlying mechanism for headaches in cervical spondylosis.
