Background:

  • Competency-based medical education (CBME) is the dominant graduate medical education model.
  • Interest in time-variable training within CBME is increasing.
  • Limited understanding exists on the impact of competency-based time-variable training (CBTVT) on learners.

Purpose of the Study:

  • To explore the impact of a CBTVT pilot program on resident motivation for learning.
  • To investigate how CBTVT affects resident engagement with assessment and feedback.
  • To understand learner experiences within the Transitioning in Internal Medicine Education Leveraging Entrustment Scores Synthesis (TIMELESS) program.

Main Methods:

  • Qualitative educational case study of the TIMELESS program (October 2020–March 2022).
  • Semistructured interviews with residents (n=9) conducted approximately every six months.
  • Inductive thematic analysis of interview data, compared with learner motivation theories.

Main Results:

  • CBTVT had variable effects on resident motivation for learning.
  • Residents showed increased engagement with and awareness of the assessment program.
  • A tension was reported between performance-driven advancement and growth-oriented learning.

Conclusions:

  • Resident motivation for learning, assessment, and feedback was impacted by CBTVT.
  • Findings are contextualized using goal orientation and attribution theories.
  • Future research should explore interventions to promote mastery learning over performance orientation.
