Chronic Obstructive Pulmonary Disease-I: Introduction01:20

Chronic Obstructive Pulmonary Disease-I: Introduction

2.9K
Chronic Obstructive Pulmonary Disease (COPD) is a long-lasting respiratory condition requiring continuous attention and care. It is a progressive lung disease that leads to breathing challenges due to airflow obstruction. It manifests as persistent respiratory symptoms and restricted airflow resulting from abnormalities in the airways and alveoli, usually due to long-term exposure to harmful particles or gases. COPD mainly consists of two primary conditions: emphysema and chronic bronchitis.

Chronic Obstructive Pulmonary Disease-IV: Assessement and Diagnostic Studies

2.6K
Assessing and diagnosing Chronic Obstructive Pulmonary Disease (COPD) involves a detailed approach that includes a comprehensive review of medical history, physical examination, and a variety of diagnostic tests. This thorough evaluation is essential to ensure an accurate diagnosis and guide effective management strategies.

Chronic Obstructive Pulmonary Disease-III: Symptoms and Complications.

2.9K
Understanding the variety of primary symptoms and systemic complications that characterize chronic obstructive pulmonary disease (COPD) is crucial for healthcare professionals.
Symptoms of COPD can be classified as primary or systemic. Primary symptoms relate to reduced airflow, while systemic or extrapulmonary symptoms relate to COPD's broader impact on the body.

Classification of Illness01:17

Classification of Illness

7.7K
The meaning of illness is individualized to each person who experiences an alteration in health. In contrast, disease is a medical term indicating a pathological change in the structure and function of the body or mind. It is a condition that has specific symptoms and boundaries.
An illness is a response to a disease in which the person's level of functioning is changed compared with a previous level. The general classification of illness includes acute and chronic.

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Summary

Pediatric chronic critical illness (CCI) lacks a clear definition, hindering research. This review found varied definitions for CCI and prolonged PICU stays, highlighting the need for a consensus to advance care.

Area of Science:

  • Pediatric critical care medicine
  • Clinical definitions and research methodology

Background:

  • Children with chronic critical illness (CCI) are a high-risk group with potential for prolonged organ dysfunction and recurrent critical care needs.
  • However, a standardized definition for pediatric CCI is currently lacking, creating challenges in research and clinical practice.

Purpose of the Study:

  • To conduct a scoping review of existing literature to identify and evaluate case definitions for pediatric chronic critical illness (CCI).
  • To examine definitions of prolonged pediatric intensive care unit (PICU) admission and the methodologies used to derive these definitions.

Main Methods:

  • A comprehensive search of four electronic databases (Ovid Medline, Embase, CINAHL, Web of Science) was performed up to March 2021.
  • A hybrid approach combining crowdsourcing and machine learning was employed for efficient and accurate screening of over 6,000 citations.
  • Studies providing specific case definitions for CCI or prolonged PICU admission were included for data extraction.

Main Results:

  • Twelve of 67 included studies (18%) offered definitions for pediatric CCI, incorporating elements like PICU length of stay, medical complexity, chronic conditions, recurrent admissions, and technology dependence.
  • Definitions for CCI were often referenced or opinion-based.
  • The majority of studies (82%) defined prolonged PICU admission, typically as 14 or 28 days or longer, with definitions derived from investigator opinion or statistical methods.

Conclusions:

  • Existing definitions for pediatric chronic critical illness are inconsistent, varying in their consideration of patient complexity and illness chronicity.
  • The lack of a consensus definition impedes research in this critical area of pediatric critical care.
  • Establishing a standardized definition for pediatric CCI is crucial for advancing research and improving patient outcomes.
