Summary

Hyperspectral imaging (HSI) offers a feasible method to quantify Raynaud phenomenon (RP) severity in systemic sclerosis (SSc) patients. While HSI shows potential for disease assessment, patient-reported outcomes did not correlate with HSI measurements.

Background:

  • Quantitative outcome measures are lacking for Raynaud phenomenon (RP) clinical trials in systemic sclerosis (SSc).
  • Hyperspectral imaging (HSI) non-invasively measures skin oxygenation, including oxygenated hemoglobin (oxyHb), deoxygenated hemoglobin (deoxyHb), and oxygen saturation (O2 sat).

Purpose of the Study:

  • To explore the potential role of HSI in quantifying SSc-RP disease severity and activity.
  • To assess HSI's feasibility in a clinical setting for SSc-RP patients.

Main Methods:

  • Prospective recruitment of SSc-RP patients (n=13) and healthy controls (HC; n=12).
  • Bilateral hand HSI performed at baseline and after a cold provocation challenge (1 min immersion in 15°C water).
  • Calculation of oxyHb, deoxyHb, and O2 sat values; completion of patient-reported outcome instruments (RCS, CHFS).

Main Results:

  • SSc patients exhibited higher baseline deoxyHb and lower O2 sat compared to HC (p < 0.05).
  • SSc patients showed a greater decline in oxyHb and O2 sat post-cold challenge with distinct rewarming trajectories (p < 0.01).
  • No significant correlations were found between HSI parameters and patient-reported outcome scores (RCS, CHFS).

Conclusions:

  • HSI is a feasible tool for SSc-RP quantification in a clinical setting.
  • HSI shows potential as a quantitative measure for SSc-RP severity and activity, particularly in response to cold provocation.
  • Longitudinal studies are needed to assess HSI's sensitivity to change over time.
