There are hollow fluid-filled cavities known as ventricles deep inside the human brain. There are two lateral ventricles, one in each cerebral hemisphere, and each has three different projections — the anterior, inferior, and posterior horns visible from the lateral side. A thin membrane called the septum pellucidum separates the two lateral ventricles. The slender third ventricle in the diencephalon is connected to each lateral ventricle via a channel called the interventricular foramen.

Summary

A rare conofilar cyst in neonates, associated with ventriculus terminalis and tethered filum, may require surgical intervention due to progressive enlargement. Early surgical treatment led to a positive outcome in this case.

Background:

  • Filar cysts are common in neonates and typically resolve naturally.
  • Ventriculus terminalis cysts are usually clinically insignificant.
  • Extra-axial cysts at the conus-filum junction are exceptionally rare, particularly when combined with tethered lipomatous filum terminale and progressive enlargement.

Observation:

  • A neonate presented with ventriculus terminalis cyst, an extra-axial cyst at the conus-filar junction, and a taut lipomatous filum.
  • Imaging revealed persistence of these findings at 6 months.
  • The extra-axial cyst enlarged after bony decompression and dural opening.

Findings:

  • The extra-axial cyst communicated with the central canal and contained neuroglial and axonal elements.
  • Surgical excision of the cyst and sectioning of the filum were performed.
  • Histological examination confirmed neuroglial and axonal components within the cyst wall.

Implications:

  • Progressive dilation of ventriculus terminalis and conofilar cysts with tethered filum terminale warrant surgical intervention.
  • This case highlights the importance of surgical management for rare spinal abnormalities.
  • Successful surgical decompression and excision resulted in a favorable long-term outcome without neurological deficits.
