Purpose of the Study:

To implement a novel virtual discharge (VDC) protocol to optimize resource allocation.
To leverage virtual care for enhanced patient education and outcomes.

Main Methods:

Established a partnership between bedside nurses and a virtual nurse team.
Utilized existing telehealth technology for remote discharge education.
Implemented the VDC protocol on a 30-bed orthopedic unit.

Main Results:

Conducted 269 VDC sessions, totaling over 101 hours of discharge education.
Achieved significant improvement in patient satisfaction communication scores.
Maintained a low 7-day patient readmission rate.

Conclusions:

The VDC model, utilizing virtual technology, proved highly satisfactory for patients and nurses.
Nurse leaders are encouraged to explore and maximize virtual technology benefits in healthcare.
