Background:

  • Encopresis, or fecal incontinence, affects children's social competence and behavior.
  • Physiologic abnormalities during defecation attempts are common in encopretic children.

Purpose of the Study:

  • To evaluate social competence and behavioral profiles in encopretic children.
  • To correlate these profiles with physiologic abnormalities and treatment outcomes.

Main Methods:

  • Assessed social competence and behavior in 38 encopretic children.
  • Performed anorectal manometry and EMG evaluations.
  • Correlated findings with defecation ability and treatment persistence.

Main Results:

  • 66% of children couldn't defecate rectal balloons; 63% couldn't relax the external anal sphincter.
  • Inability to defecate or relax the sphincter predicted encopresis persistence.
  • Social competence was lower in girls unable to defecate balloons.

Conclusions:

  • Physiologic defecation abnormalities, not social or behavioral scores, predict encopresis persistence.
  • Targeting defecation dysfunction is crucial for treating encopresis.
