Background:

  • Extracting impacted maxillary third molars presents surgical challenges including limited visibility and risk of complications.
  • A novel buccal rotation technique is proposed to address these difficulties.

Purpose of the Study:

  • To introduce and evaluate the clinical applicability of the buccal rotation method for extracting impacted maxillary third molars.
  • To compare the efficacy of the buccal rotation technique against traditional methods.

Main Methods:

  • A cohort study included 72 impacted maxillary third molars between October 2020 and September 2021.
  • Teeth were classified by impaction position (I, II, III) and angulation (A, B, C) relative to adjacent teeth.
  • Surgical simulations determined the application of traditional or buccal rotation methods; results were analyzed using the Chi-square test.

Main Results:

  • The buccal rotation method was predominantly used for deeply impacted teeth (position III) and those with significant angulation (angulation C) (p<0.05).

  • Four temporary complications were reported across all cases.

Conclusions:

  • Buccal rotation is a feasible and effective technique for extracting deeply impacted maxillary third molars.
  • The method is particularly advantageous for molars with substantial angulation towards adjacent teeth.
