Optic perineuritis (OPN) is a newly identified complication of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. This case highlights a potential autoimmune response to SARS-CoV-2, leading to neurological sequelae like OPN.
Area of Science:
- Neurology
- Infectious Diseases
- Ophthalmology
Background:
- Optic perineuritis (OPN) is a rare neurological complication.
- Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can lead to various neurological sequelae.
- This case presents OPN following SARS-CoV-2 infection, even in asymptomatic individuals.
Observation:
- A 71-year-old male with NIDDM tested positive for SARS-CoV-2 RNA.
- Approximately 3 weeks post-infection, he developed ophthalmoparesis, diplopia, and pain.
- MRI suggested OPN; cerebrospinal fluid analysis ruled out other causes.
Findings:
- The patient received high-dose intravenous and oral steroids.
- Within 16 days of treatment, his symptoms significantly improved.
- This suggests a positive response to immunosuppressive therapy.
Implications:
- SARS-CoV-2 can cause diverse neurological manifestations beyond respiratory symptoms.
- Post-infectious OPN may be an autoimmune-mediated response to SARS-CoV-2.
- Early recognition and treatment of OPN in SARS-CoV-2 patients are crucial.
