Area of Science:

  • Thoracic surgery
  • Surgical oncology
  • Robotic surgery

Context:

  • The Japan Clinical Oncology Group (JCOG) 0802 study is increasing the role of segmentectomy for lung cancer.
  • Robot-assisted thoracic surgery (RATS) provides enhanced precision for complex dissections.
  • Segmentectomy is a lung-sparing surgical procedure for early-stage lung cancer.

Purpose:

  • To evaluate the feasibility and outcomes of robot-assisted segmentectomy for lung cancer.
  • To compare robot-assisted segmentectomy with traditional thoracoscopic approaches.
  • To assess the impact of RATS on operating time, complications, and patient recovery.

Summary:

  • Between April 2020 and April 2022, 52 lung cancer segmentectomies were performed, with 8 (15.4%) utilizing RATS.
  • RATS procedures had a median operating time of 233 minutes, significantly longer than thoracoscopic segmentectomy (184 minutes, p=0.007).
  • No significant differences were observed in drain removal time, complication rates, or postoperative pain scores between RATS and thoracoscopic surgery.

Impact:

  • Robot-assisted surgery offers improved visualization and precision, potentially benefiting patients.
  • Despite longer operating times, RATS may offer advantages with evolving technology and techniques.
  • Further research and technological advancements are expected to enhance the benefits of RATS in lung cancer treatment.
