Monkeypox can present atypically, including as proctitis, diverging from historical rash patterns. Early recognition and treatment are crucial for managing this evolving zoonotic disease.
Area of Science:
- Infectious Diseases
- Dermatology
- Public Health
Background:
- The 2022 human monkeypox virus outbreak presented with atypical symptoms beyond the historically recognized rash illness.
- Previous cases typically showed rashes on the face, palms, and soles, with nonspecific symptoms like fever and lymphadenopathy.
Observation:
- A case report details a 32-year-old male with HIV experiencing worsening rectal pain and diffuse skin lesions, including on hemorrhoids.
- Imaging revealed rectal thickening consistent with proctitis, and PCR testing confirmed monkeypox infection.
Findings:
- The patient received a 14-day course of tecovirimat for disseminated monkeypox infection.
- The patient showed improved pain control and was discharged after initiating antiviral treatment.
Implications:
- Clinicians must consider monkeypox in the differential diagnosis for proctitis due to emerging atypical presentations.
- Further development of evidence-based treatment protocols is needed for managing monkeypox-associated proctitis and other atypical manifestations.

Acute Pancreatitis II: Clinical Manifestations and Management
Acute Pancreatitis I: Introduction
Acute pancreatitis is characterized by rapid inflammation of the pancreas, often caused by factors like gallstone blockage or excessive alcohol consumption. Chronic pancreatitis, on the other hand, is a slow, progressive inflammation that may result from long-term alcohol abuse, obstructions in the pancreatic duct, or genetic factors.
The causes of acute pancreatitis include:

Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
Chronic Pancreatitis I: Introduction
Pancreatitis is the inflammation of the pancreas, which occurs when the immune system becomes active and causes swelling, pain, and disruptions in organ function. Pancreatitis can manifest as either an acute or chronic condition.
Acute pancreatitis arises suddenly and lasts for a brief duration, while chronic pancreatitis is a long-term affliction...
