Background:

  • Apical hypertrophic cardiomyopathy (AHC) is a nonobstructive form of hypertrophic cardiomyopathy.
  • AHC involves disproportionate thickening of the left ventricular apex.

Observation:

  • AHC is increasingly diagnosed, posing challenges for airmen's flying status.
  • A case study of a pilot with AHC over 15 years, remaining asymptomatic, is presented.

Findings:

  • A spectrum of AHC severity exists, from mild to severe.
  • Mild AHC may permit restricted aviation licensing.

Implications:

  • Consideration for restricted pilot licensing requires a normal exercise test and no significant arrhythmias.
  • Regular cardiological follow-up (every 6 months) is essential for licensed pilots with mild AHC.
