Background:

  • Patient interest in waterbirth is growing, yet robust evidence on its outcomes is scarce.
  • Existing literature lacks comprehensive comparisons of maternal and perinatal results between waterbirth and traditional landbirth.

Purpose of the Study:

  • To systematically review and compare maternal and perinatal outcomes for waterbirth versus landbirth.
  • To synthesize evidence from randomized controlled trials to inform clinical practice and patient choice.

Main Methods:

  • A comprehensive literature search was conducted across multiple databases (MEDLINE, Scopus, Cochrane Library, etc.) up to November 2021.
  • Included randomized controlled trials (RCTs) comparing waterbirth with landbirth in women with singleton cephalic gestations (≥36 weeks).
  • Data were analyzed using random-effects models, reporting relative risks and mean differences with 95% confidence intervals.

Main Results:

  • Waterbirth was associated with significantly lower rates of labor augmentation and neuraxial anesthesia.
  • Maternal satisfaction was higher, and pain scores were lower with waterbirth. Labor durations (first, third, and overall) were also shorter.
  • No significant differences were found in estimated blood loss, postpartum hemorrhage, perineal lacerations, episiotomy rates, or mode of delivery. Neonatal outcomes and specific infections like chorioamnionitis and endometritis were not adequately reported.

Conclusions:

  • Waterbirth appears to enhance maternal comfort and satisfaction while potentially shortening labor, with reduced need for interventions like anesthesia.
  • Current evidence is limited, particularly concerning neonatal safety and specific maternal infectious outcomes.
  • Further high-quality research is required to fully ascertain the safety profile and benefits of waterbirth for both mothers and neonates.
