This case report highlights a rare presentation of secondary syphilis mimicking lymphoma. Prompt diagnosis requires considering syphilis in patients with unusual nodular skin lesions and lymphadenopathy.

Background:

  • Unusual presentations may mimic other conditions, complicating diagnosis.

Observation:

  • A patient presented with a rare nodular eruption, pruritus, and generalized lymphadenopathy.
  • Histopathology initially suggested lymphocytoma cutis, further obscuring the diagnosis.

Findings:

  • Serologic tests for syphilis revealed a high VDRL titer.
  • Dark-field microscopy confirmed spirochetes in a nodular lesion, establishing the diagnosis.

Implications:

  • Secondary syphilis should be included in the differential diagnosis for nodular lesions resembling lymphoreticular disease.
