Background:

Abdominal paracentesis is a common procedure for ascites management in patients with liver cirrhosis.
Potential complications include fluid leak, infection, and hematoma.

Observation:

A 58-year-old male with decompensated liver cirrhosis developed sudden scrotal and penile swelling 12 hours post-paracentesis.
Other causes of genital swelling were excluded.

Findings:

The genital swelling was diagnosed as a rare complication of paracentesis.
Conservative management with diuretics and scrotal elevation led to symptom improvement within 2 days.

Implications:

This case highlights a rare but significant complication of paracentesis.
Understanding the potential for post-procedural genital edema is crucial for patient care.
Hypothesized mechanism involves a peritoneal-fascial tract facilitating fluid migration.
