A Rare Case of Complicated Emphysematous Pyelonephritis

• The patient also had a large 5.6 cm bladder stone and severe right-sided hydroureteronephrosis.
• Initial conservative management with bilateral nephrostomy tubes, perinephric drainage, and antibiotics failed to resolve symptoms.

Findings:

• Despite initial medical management, the patient's symptoms persisted, indicating the need for more aggressive treatment.
• Surgical intervention, including open cystolitholapaxy to remove the bladder stone and left nephrectomy, was ultimately performed.
• The combined surgical approach led to the resolution of the patient's symptoms.
