Methods of Documentation III: PIE01:21

Methods of Documentation III: PIE

1.4K
Problem-intervention-evaluation (PIE) is a systematic approach to documentation used in healthcare settings for clinical decision-making and patient care planning. It is a structured approach to organizing patient data based on problems, interventions, and evaluations. Here's a breakdown of its key features and considerations:

Apical-Radial (A-R) Pulse Assessment
The A-R pulse assessment involves simultaneous evaluation of the apical and radial pulses. When the apical and radial pulse rates vary, this assessment helps identify a pulse deficit.

Methods of Documentation VI: Case Management Model

612
The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Nursing Assessment01:29

Nursing Assessment

7.8K
The two sources for collecting information are primary and secondary. After gathering information, interpretation and validation help to complete the data. The purpose of assessment is to establish data with the initial information, to interpret data about the patient's perceived needs and health problems, and to respond to these problems identified.
The nurse collects all aspects of the patient's health in the initial assessment, establishing priorities for ongoing focused assessments...

Area of Science:

  • Nursing Informatics
  • Healthcare Management
  • Clinical Assessment Tools

Background:

  • Effective patient acuity tools are crucial for optimal nurse staffing and patient care assignments.
  • Evaluating the impact of new tools on nursing practice is essential for successful implementation.
  • Part-time and full-time Registered Nurses (RNs) have distinct workflow needs that may influence tool perception.

Purpose of the Study:

  • To assess the acceptability, relevance, and effectiveness of a patient acuity tool (PAT) among part-time and full-time RNs.
  • To determine if the PAT improves the accuracy of patient acuity assessment and aids in nurse-patient assignments.

Main Methods:

  • A pre- and post-test intervention design was employed on a 28-bed telemetry unit at a Magnet®-designated hospital.
  • An eight-item questionnaire measured nurse acceptability of the PAT.
  • RNs' opinions on the relevance and effectiveness of the PAT were formally evaluated.

Main Results:

  • A 66.6% participation rate (22/33 RNs) was achieved for the PAT questionnaire.
  • Statistically significant improvements were observed in two key areas post-intervention.
  • RNs reported that the PAT significantly assisted charge nurses in making nurse-patient assignments (t=3.070, P=.006) and improved the accuracy of acuity assessment (t=2.390, P=.026).

Conclusions:

  • Patient acuity tools should be tailored to specific nursing units and adaptable to dynamic staffing and patient needs.
  • Consideration of factors like patient condition changes, admissions/discharges, and psychosocial elements is vital for tool effectiveness.
  • The PAT demonstrated potential to enhance nursing workflow and patient assignment accuracy in a telemetry setting.
