Background:

  • Heparin-induced thrombocytopenia necessitates alternative anticoagulants.
  • Dabigatran shows promise as an anticoagulant for CPB.

Purpose of the Study:

  • To test the hypothesis that dabigatran provides sufficient anticoagulation for CPB in rabbits.
  • To compare dabigatran's efficacy against heparin in a rabbit CPB model.

Main Methods:

  • Nonlinear mixed effects models estimated dabigatran pharmacokinetics in rabbits.
  • Dabigatran infusion maintained target plasma concentrations during CPB.
  • Activated clotting time and thromboelastometry assessed anticoagulation.
  • Histology and electron microscopy evaluated for thrombosis and damage.

Main Results:

  • Dabigatran concentrations exceeded targets but prevented thrombosis during 2-hour CPB.
  • Both dabigatran and heparin showed elevated reaction times.
  • No significant differences in thrombosis or organ damage were observed between groups.
  • Arterial filters showed minimal fibrin in both groups.

Conclusions:

  • Dabigatran provides acceptable anticoagulation for CPB in a rabbit model.
  • Dabigatran is a potential alternative to heparin for CPB.
  • This study is a proof-of-concept for dabigatran use in CPB.
