Background:

  • A 62-year-old male with medically uncontrolled glaucoma underwent bilateral trabeculectomy.
  • The patient had a history of gastric cancer requiring total gastrectomy (TG).

Observation:

  • Following TG and chemotherapy (S-1+CDDP, weekly PAC, CPT-11), the patient experienced substantial weight loss (BMI decreased from 29.5 to ~19).
  • Intraocular pressure (IOP) in both eyes decreased significantly after TG and chemotherapy, despite previous challenges with eye drop control.

Findings:

  • The patient's IOP, previously difficult to manage with medications, decreased to 6-10 mmHg (right eye) and 8-15 mmHg (left eye) post-treatment.
  • The decrease in IOP correlated with significant body mass index reduction following the cancer interventions.

Implications:

  • Total gastrectomy and associated chemotherapy may influence intraocular pressure in glaucoma patients.
  • Further research is warranted to explore the mechanisms linking cancer treatment, weight loss, and IOP reduction in glaucoma.
