Severe leptospirosis in Thailand can be predicted using six key indicators, including hemoptysis and hypotension. These findings can help develop a scoring system to identify high-risk patients and improve outcomes.
Area of Science:
- Infectious Diseases
- Clinical Medicine
- Epidemiology
Background:
- Leptospirosis poses a significant public health challenge in Thailand, with varying disease severity.
- Early identification and management of severe leptospirosis are critical to prevent complications and reduce mortality.
- This study aimed to identify predictors for severe leptospirosis in clinical practice.
Purpose of the Study:
- To explore parameters for predicting severe leptospirosis illness.
- To identify key clinical and laboratory indicators associated with severe disease.
- To provide a basis for developing a predictive scoring system.
Main Methods:
- A case-control study was conducted across eight general hospitals in Thailand.
- Retrospective data were collected from inpatient medical records.
- A multivariate model was developed using backward elimination to identify significant predictors.
Main Results:
- Six significant predictors for severe leptospirosis were identified: hemoptysis, hypotension, low platelet count (<100,000/µL), high white blood cell count (>14,000/µL), low hematocrit (≤30%), and jaundice.
- These predictors demonstrated a high accuracy in predicting severe leptospirosis, with an area under the receiver operation curve (AuROC) of 91.31%.
- Odds ratios ranged from 3.11 for jaundice to 25.80 for hemoptysis, indicating strong associations with severe disease.
Conclusions:
- Severe leptospirosis infections exhibit identifiable predictors in a clinical setting.
- The identified predictors can be utilized to create a scoring system for assessing disease severity.
- This tool could aid clinicians in risk stratification and timely intervention for leptospirosis patients.
