Background:

Accurate breast cancer risk stratification is crucial for effective screening and prevention.
Existing models like BRCAPRO (BRCApro) and BCRAT (Gail model) have limitations, often focusing on either genetic or non-genetic factors.
Integrating diverse risk factors can enhance prediction accuracy.

Purpose of the Study:

To improve breast cancer risk prediction by combining the BRCAPRO and BCRAT models.
To evaluate two novel approaches for integrating these models: penetrance modification and ensemble modeling.
To assess the performance of combined models against individual models and existing integrated approaches.

Main Methods:

Two methods were used to combine BRCAPRO and BCRAT: modifying BRCAPRO penetrance functions with BCRAT hazard estimates, and training an ensemble model using both predictions.
The models were validated using simulated data and real-world data from Newton-Wellesley Hospital and the Cancer Genetics Network.
Performance was compared to BRCAPRO, BCRAT, and the IBIS model.

Main Results:

Combined models demonstrated improved performance over individual BRCAPRO and BCRAT models.
The BRCAPRO + BCRAT penetrance modification approach showed comparable performance to the IBIS model in the Cancer Genetics Network cohort.
Integration effectively leverages both genetic and non-genetic risk factors for more accurate predictions.

Conclusions:

Combining BRCAPRO and BCRAT offers a valuable strategy for enhancing breast cancer risk prediction accuracy.
The developed integration methods provide a more comprehensive assessment of individual cancer risk.
Improved risk stratification can lead to more personalized and effective cancer screening and prevention strategies.
