Background:

  • Breast tuberculosis (BTB) is an uncommon form of extrapulmonary tuberculosis (TB).

Observation:

  • A 36-year-old woman with a history of breast reduction surgery presented with worsening breast enlargement, pain, purulent discharge, fever, and abdominal pain.
  • Imaging revealed a large fluid collection with abscesses, adenopathies, and fistulae.
  • Initial cultures were sterile, but lymph node biopsy identified *Mycobacterium tuberculosis* (Mt).

Findings:

  • The case highlights the diagnostic challenges of BTB, which can mimic bacterial abscesses or malignancy.
  • Cultures from drained fluid were negative for common pathogens, delaying definitive diagnosis.
  • Histopathology and culture from lymph node biopsy confirmed *Mycobacterium tuberculosis*.

Implications:

  • A high index of suspicion for BTB is essential in endemic areas, especially with atypical presentations.
  • Accurate and timely diagnosis of BTB is critical for appropriate antimicrobial therapy and patient management.
  • This case underscores the importance of considering rare infections in differential diagnoses of complex breast conditions.
