Hallux Valgus Interphalangeus with Large Ossicle Formation: A Case Report

Hallux valgus interphalangeus (HVIP), a great toe deformity, can be effectively treated with Akin osteotomy and ossicle excision. This surgical approach improved joint angle and patient satisfaction in a complex case.

Background:
- Hallux valgus interphalangeus (HVIP) is a deviation of the distal phalanx from the proximal phalanx.
- Its multifactorial etiology includes developmental disturbances, external pressures, and joint biomechanics.
- Ossicles, bony growths, can be associated with foot deformities.
Observation:
- A case of HVIP in a 21-year-old woman with childhood onset is presented.
- The patient experienced pain exacerbated by walking and footwear.
- A large lateral ossicle was noted and considered a contributing factor to the HVIP.
Findings:
- Surgical correction involved Akin osteotomy, headless screw fixation, ossicle excision, and medial capsulorrhaphy.
- The interphalangeal joint angle improved significantly, from 28.69° pre-operatively to 8.93° post-operatively.
- The patient reported satisfaction with the surgical outcome and uneventful wound healing.

- Akin osteotomy combined with ossicle excision proved effective for this HVIP case.
- Understanding the role of ossicles in foot deformities is crucial for surgical planning.
- Further research on foot ossicles can enhance the biomechanical understanding of deformity correction.
