Background:

  • Current endoscopic grading scales inadequately assess postoperative nasal polyposis.
  • Accurate assessment of polyp recurrence is crucial for managing chronic rhinosinusitis with nasal polyps (CRSwNP).

Purpose of the Study:

  • To develop and validate a novel endoscopic grading system, the Postoperative Polyp Scale (POPS).
  • To improve the characterization of polyp recurrence in postoperative paranasal sinus cavities.

Main Methods:

  • A modified Delphi method involving 13 otolaryngologists, rhinologists, and allergists established the POPS.
  • Fellowship-trained rhinologists reviewed 50 postoperative endoscopy videos from CRSwNP patients using the POPS.
  • Test-retest and inter-rater reliability were assessed for the POPS.

Main Results:

  • The POPS demonstrated good inter-rater reliability (Kappa = 0.49-0.50).
  • Excellent test-retest reliability was observed (Kappa = 0.80).

Conclusions:

  • The POPS is a reliable and objective endoscopic grading scale for postoperative polyp recurrence.
  • This novel scale can aid in measuring the efficacy of medical and surgical interventions for CRSwNP.
