Purpose of the Study:

  • To investigate the association between posterior staphyloma (PS) and the development of myopic maculopathy.
  • To identify the key clinical and anatomical factors associated with the presence of PS in highly myopic eyes.

Main Methods:

  • A cross-sectional study involving 467 highly myopic eyes (axial length [AL] ≥26 mm) from 246 patients.
  • Ophthalmological examinations and multimodal imaging were performed.
  • Eyes were analyzed for the presence of PS and compared based on age, AL, best-corrected visual acuity (BCVA), atrophy/traction/neovascularization (ATN) components, and severe pathologic myopia (PM) in age-matched and AL-matched cohorts.

Conclusions:

  • Posterior staphyloma (PS) is strongly associated with the presence of myopic maculopathy and reduced visual acuity.
  • The development of PS is primarily driven by axial length and age, in that order.
  • These findings underscore the importance of monitoring PS in highly myopic patients to mitigate risks of severe visual impairment.
