Hepatitis B surface antigen (HBsAg) was not found in kidney tissue of seropositive glomerulonephritis patients. A false positive staining in seronegative patients highlights diagnostic challenges.

    Area of Science:

    • Nephrology
    • Immunology
    • Virology

    Background:

    • Glomerulonephritis is a kidney disease affecting the glomeruli.
    • Hepatitis B virus (HBV) infection is a global health concern.
    • The potential link between HBV and glomerulonephritis requires investigation.

    Purpose of the Study:

    • To investigate the presence of Hepatitis B surface antigen (HBsAg) in kidney biopsies of patients with glomerulonephritis.
    • To determine if HBsAg deposition in renal tissue is associated with glomerulonephritis.
    • To evaluate the reliability of immunofluorescence staining for HBsAg in kidney biopsies.

    Main Methods:

    • Studied 97 patients with various forms of glomerulonephritis.
    • Assessed serum for Hepatitis B surface antigen (HBsAg) seropositivity.

    Related Experiment Videos

  • Performed direct immunofluorescence kidney staining for HBsAg.
  • Investigated potential sources of false positive staining.
  • Main Results:

    • 10 out of 97 patients were HBsAg seropositive; none had HBsAg kidney deposits.
    • 4 out of 87 HBsAg seronegative patients showed HBsAg staining in kidney tissue.
    • This staining was identified as a false positive reaction.
    • The false positive reaction resulted from Fc portion binding to IgM rheumatoid factor (RF) tissue deposits.
    • Standard controls did not detect this false positive HBsAg staining.

    Conclusions:

    • HBsAg deposition in kidney tissue was not confirmed in HBsAg-seropositive glomerulonephritis patients.
    • A novel false positive immunofluorescence staining mechanism for HBsAg in kidney tissue was identified.
    • The role of HBsAg in the pathogenesis of glomerulonephritis remains unproven based on this study.
