Electronic nicotine delivery systems (ENDS) use did not significantly increase chronic obstructive pulmonary disease (COPD) risk in adults aged 40+ when accounting for cigarette smoking history. Cigarette pack years remained a significant risk factor for COPD incidence.

- Adequate adjustment for cigarette smoking history is essential for accurately assessing the independent health effects of ENDS.
- Cigarette smoking remains a critical risk factor for COPD development.
