Pediatric endocarditis, though rare, can cause shortness of breath on exertion in children. Point-of-care ultrasound aided in the early diagnosis of ventricular septal defect with tricuspid valve endocarditis.

Observation:
- A 7-year-old male presented with nocturnal fever, malaise, and fatigue after a viral illness.
- The patient experienced shortness of breath on exertion (SOBOE).
Findings:
- Point-of-care ultrasound (POCUS) initially suggested a ventricular septal defect (VSD) with tricuspid valve (TV) endocarditis.
- Formal echocardiography confirmed the diagnosis of VSD with TV endocarditis.
Implications:
- POCUS facilitated early suspicion and diagnosis of pediatric endocarditis.
- This led to expedited antibiotic treatment and blood cultures, improving patient management.
